PCN accepts new applications only during open enrollment periods. Dates to be announced.

♦ Age 19 through 64 with one or more children
♦ High cost or no insurance offered through employer
♦ No full time students
♦ Income limits (see table below).

Enrollees need to go to orientation.
The Bureau of Eligibility Services has a Medicaid Eligibility Worker who speaks English and Spanish. His office is located in the Community Health Center building. He determines eligibility for Medicaid programs (family, aged, disability) as well as CHIP and PCN eligibility. He does not determine food stamp eligibility. For more information, call 652-4072.
