Self Pay / No Insurance
The Community Health Center accepts patients with no insurance or who are self pay. We offer reduced rates (on a sliding fee scale) based on your family size and income.
As an example, if you have 4 family members and your family income is below $20,650 a year, you would pay $20 per visit. If you have 4 family members and your family income is between $20,651 and $30,975 you would pay $35 per visit.
We'll need you to bring proof of income on your first visit so that we can determine where you fall on the sliding fee scale.
We offer same day or next day appointments, which makes seeing a doctor very convenient when you need one!
As a patient, you are eligible to participate in our dental voucher program. You can receive up to $250 worth of dental services for only $25. (Funded through state and private grants).
We can also help get you reduced precription pricing.
