Financial Information
Fees at Southwest Utah Community Health Center are comparable to the fees charged by other health care providers in our area.
Reduced fees are available for non-insured, under-insured and other eligible patients based on family size and household income. The reduced fees are determined by a sliding discount fee schedule.
Payment plans are available.
No patient will be denied access to care due to inability to pay full fees.
Clients with Medicare, Medicaid, CHIP, PCN and other insurances are welcome.
The continued operation of our health center depends upon the collection of fees for our services. The ability to offer discounted rates depends on several sources of revenue:

☻ Federal and private grants
☻ gifts and donations
