Services
Family Dental Plan
Southwest Utah Community Health Center has a contract with the Utah Department of Health's Family Dental Plan (Medicaid dentist). Through this arrangement, uninsured Community Health Center patients can be referred to the Washington County Family Dental Plan office. The Community Health Center reimburses the Family Dental Plan at the Medicaid rate on a fee for service basis.
Due to the demand for dental services, the Community Health Center has had to limit the number of dental referrals made each month. Thanks to generous donations from The Salvation Army, Dixie Ladies of Elks, Intermountain Community Partners, Dolores Doré Eccles Foundation, and the Utah Department of Health, the Community Health Center is able to provide dental care services to many more individuals than our funds alone would allow.
