PCN

Applications are now being accepted for the Children's Health Insurance Program (CHIP).  CHIP will remain open until further notice.

CHIP is a state health insurance plan for children.  Many working Utah families who don't have other health insurance qualify for CHIP.

What does CHIP cover?

☺  Immunizations
☺  Health care provider visits
☺  Health care provider services
☺  Prescriptions
☺  Hearing and eye exams
☺  Mental Health Services
☺  Dental services for prevention and treatment of tooth decay

Help spread the word about open enrollment for CHIP.

The Bureau of Eligibility Services has a Medicaid Eligibility Worker who speaks English and Spanish.  His office is located in the Community Health Center building.  He determines eligibility for Medicaid programs (family, aged, disability) as well as CHIP and PCN eligibility.  He does not determine food stamp eligibility.  For more information, call 652-4072.
