Applications are now being accepted for the Children's Health Insurance Program (CHIP). CHIP will remain open until further notice. CHIP is a state health insurance plan for children. Many working Utah families who don't have other health insurance qualify for CHIP. What does CHIP cover?
Parents, grandparents, teachers and others who care for children are encouraged to learn more about CHIP at the following website: or by calling 1-877- KIDS-NOW Help spread the word about open enrollment for CHIP. The Bureau of Eligibility Services has a Medicaid Eligibility Worker who speaks English and Spanish. His office is located in the Community Health Center building. He determines eligibility for Medicaid programs (family, aged, disability) as well as CHIP and PCN eligibility. He does not determine food stamp eligibility. For more information, call 652-4072.
