Services
Southwest Utah Community Health Center is a non-profit, community-based corporation providing comprehensive primary and preventive health care and behavioral health services to individuals and families at reduced rates, based on your family size and household income. No one is denied access to care due to their inability to pay full fees at the time of their visit.
We provide medical care, dental vouchers, prenatal care, deliver babies, provide well child care, women's health care, chronic disease care and have a mental health counselor on site. Our staff includes a family practice physician and two physician assistants. We strive to meet the unique health needs of the community in linguistically and culturally appropriate ways. We accept all insurance plans, Medicaid, Medicare, CHIP, PCN and work on a sliding fee scale for uninsured patients. We have reduced price prescription medications available as well.
If you call for an appointment you will usually be seen today or tomorrow as we are open from 8 am to 7 pm Monday through Friday.
